Notice of Privacy Practices

Effective Date: 01 February 2008

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED. PLEASE REVIEW IT CAREFULLY.

EMED International ("eMED") is an organization that standardizes methods of communicating critical medical information and informs the public and healthcare industry about using these processes. EMED is not a health plan, health care clearinghouse, or health care provider. EMED is dedicated to maintaining the privacy of your individual health information.

The health information you submit via this website will be used to make a medical information card for you, also known as an "eMED Card." The eMED Card is designed to carry the pertinent health information a physician or first responder would need to treat you in an emergency situation. EMED never receives, has access to, or alters in any way the health information you submit. Once submitted, your health information is immediately electronically stored on a secure server. Information stored in our database is considered confidential, and is not publicly accessible, and will not be divulged to any individual not employed by EMED or E.S. Systems, Inc., the company contracted by us to maintain this website or Emrys, the company contracted by us to host this website or EDCO, the company contracted by us to produce the eMED Card. Emrys never views, or alters your health information. Emrys automatically, electronically and securely, transfers your health information to a separate company called EDCO. EDCO uses the health information you provide for the sole purpose of producing your eMED Card. You will be able to access your health information via this website in order to update the information and make any necessary changes. However, no other parties, including EDCO, E.S. Systems, Inc., Emrys, and your insurance provider, will have access to this information.

The shopping cart page contains information about products to be ordered, and is where product quantities can be altered, and undesired products can be removed. The order information page contains fields for entering shipping and billing information, including your name, address, and e-mail address. The payment information page contains fields for entering your credit card information, and serves as our authorization to charge your card for the total amount of the contents of your cart plus shipping charges, and if applicable, state sales taxes. This information is collected voluntarily, and is required in order for us to effectively process your order. Other information we request is optional.

EMED protects user information via SSL encryption during collection of health information and payment information, as well as during the payment authorization process. All collected information that is stored, which at this time consists of information collected from the eMED Form and the product order information page, is stored in a secure database that is only accessible to employees of Emrys or E.S. Systems, Inc., the companies contracted by EMED to host and maintain this website. Your payment information, which includes your credit card information, is not stored.

EMED does not pass on or disclose your health information collected via this website to any other parties except as indicated herein for the purpose of producing your eMED Card.

EDCO has privacy and security policies and procedures in place to protect your health information. These policies and procedures state that EDCO treats all records and information provided to it as confidential information; requires its employees to sign a legally binding confidentiality statement; trains employees on how to comply with security and privacy provisions; has termination procedures that insure that employees who leave EDCO no longer have access to records; has a specific individual responsible for monitoring security procedures; and, as of the Effective Date of this Notice of Privacy Practices, has never had a security breach with unauthorized access to records.

EMED reserves the right to revise or amend this Notice at any time. Should our privacy practices change, we will post a revised Notice of Privacy Practices on this website.

If you believe your privacy rights have been violated, you may file a complaint with our company or with the Secretary of the Department of Health and Human Services. There will be no retaliation for filing a complaint.
